How to itemise Carmilla and, more importantly, why each item earns its slot.

Overview

Why this build works on Carmilla

Carmilla excels at frontlining and setting up teamfights through her ultimate, Curse of Blood, which links enemies together to share damage and crowd control. Because she must dive directly into the enemy team to maintain Crimson Flower hits and steal defense with Vampire Pact, her build focuses heavily on extreme durability and sustain. This setup turns Carmilla into an unkillable disruption vessel. By pairing movement speed with dense health and defense stats, she can close gaps using Bloodbath, sustain through close-quarters combat with Crimson Flower healing, and hold the frontline long enough for her allies to capitalize on linked enemies.

Item by item

Tough Boots - Encourage

Early

Tough Boots - Encourage provides Carmilla with initial mobility needed to land Bloodbath stuns and engage early fights around the Turtle. The passive buff boosts nearby allies, while the base magic defense and resilience help her withstand early magic poke and crowd control when diving in to stack Vampire Pact.

In-game passive Unique Passive - Passive - Encourage: Success

Cursed Helmet synergizes directly with Carmilla's close-range playstyle. Since Crimson Flower requires her to continuously hug enemy heroes, the passive magic damage over time adds steady pressure. Furthermore, when Curse of Blood links multiple targets, burning nearby enemies amplifies the shared damage output across the linked group while enhancing her laning and wave management capabilities.

In-game passive Unique Passive - Burning Soul: Deals Magic Damage equal to 0.6% of the hero's Max HP per second to nearby enemies. This damage is increased by 140% to 350% against creeps and minions (scales with level).

Dominance Ice is essential for Carmilla when frontlining against physical threats and high-regen enemies. The attack speed reduction neuters physical marksmen and fighters who attempt to trade with her. Reducing enemy healing ensures that Carmilla and her team can out-sustain opponents while she steals physical defense using Vampire Pact and maintains her sustain with Crimson Flower.

In-game passive Unique Passive - Arctic Cold: Reduces the Attack Speed of nearby enemy heroes to 70% of normal.

Guardian Helmet boosts Carmilla's massive HP pool, directly increasing the damage output of Cursed Helmet. Its continuous HP regeneration allows her to quickly heal back to full health between skirmishes without needing to recall, enabling non-stop pressure across the map and ensuring she is always ready to initiate with Curse of Blood.

In-game passive Unique Passive - Recovery: Regenerates 2.5% Max HP per second (reduced to 0.5% Max HP per second over 5s after taking damage).

Athena's Shield

Situational

Athena's Shield grants Carmilla massive magic damage reduction, protecting her against burst mages who attempt to delete her before she can close the distance with Bloodbath. This damage mitigation allows her to safely walk into enemy spell rotations, absorb critical skill shots, and successfully cast Curse of Blood during tight jungle choke points.

In-game passive Unique Passive - Shield: Upon taking Magic Damage, gains 25% Magic Damage Reduction for 3s (applied before taking damage). Regains this effect 5s after leaving combat.

Immortality serves as the ultimate safety net for late-game teamfights. When Carmilla aggressively initiates with Bloodbath and links enemies with Curse of Blood, she is often targeted by the entire enemy team. Reviving after death gives her a second chance to cast Crimson Flower, stack Vampire Pact again, and continue controlling the battlefield while her allies finish off the linked targets.

In-game passive Unique Passive - Immortal: Revives 2.5s after death, gaining 16% Max HP and a 220-1200 shield (scales with level) that lasts 3s (210s cooldown).

Adapting the build

No build survives every draft. These are the situations where the order above should change.

Against heavy magic burst damage

Prioritize building Athena's Shield earlier in your item progression right after your core movement item. This protects Carmilla from getting melted before she can dive into the enemy backline with Bloodbath and link targets using Curse of Blood, ensuring she survives the initial burst to steal defense with Vampire Pact.

When enemies rely on high life steal or attack speed

Rush Dominance Ice immediately after completing Tough Boots - Encourage. The aura cripples basic-attack reliant heroes and cuts down enemy healing, making Carmilla's Crimson Flower HP recovery significantly more effective by comparison while allowing her to safely stack Vampire Pact against aggressive physical threats during early teamfights.

Item recommendations come from the game's own hero data. The explanations are AI-generated from Carmilla's ability kit and the items' in-game passives, and are checked automatically against the item list — they never introduce an item the game does not recommend for this hero.
